How to reduce no-shows: reminders, advances and booking rules that work

A no-show costs twice: the fee you don't earn and the slot someone else could have had. Here is what the evidence says works, what no-shows cost your business in rupees, and the five booking rules that close the gap.

Written by , Managing PartnerPublished Updated How we research our guides

The best-evidenced fix is a reminder. A Cochrane review of trials found text reminders raised attendance at health appointments compared with no reminder, and worked about as well as phone calls for less money. Add a small advance for long or first appointments, a written confirmation at booking, and a one-message way to reschedule, so people cancel instead of disappearing.

What has been measured

Do reminders reduce no-shows? What the evidence says

Yes. A Cochrane systematic review of eight randomised trials, covering 6,615 people, found that text message reminders raised attendance at healthcare appointments compared with no reminder, and did about as well as phone call reminders. The evidence is rated low to moderate quality, and it is from healthcare, but nothing better exists on the question.

From Gurol-Urganci and colleagues, Cochrane, 2013, read on 24 October 2026. Part of our guides to leads and bookings.

1.14risk ratio for attendance, text reminder vs none: seven studies, 5,841 people
0.99text vs phone call: about the same effect, three studies, 2,509 people
55-65%lower cost per attendance for a text than a phone call, in two studies

A risk ratio of 1.14 means the attendance rate with a reminder was 1.14 times the rate without one. How much that is worth to you depends on how many people miss appointments now, which is why the calculator below uses your numbers rather than the trials'.

What no-shows cost you a month, and what an advance recovers

Enter your bookings a week, how many people usually miss, your fee and the advance you would ask. The calculator shows the fees lost to no-shows each month, how much an advance would keep, and what automatic WhatsApp reminders would cost at Meta's published rate for India. It runs in your browser.

Kept when someone doesn't come, if your terms say so.
No-shows a month17Out of about 173 bookings.
Fees lost a month₹10,200
Kept by the advance₹3,400From the people who still don't come.
Automatic WhatsApp reminders₹20One reminder per booking at ₹0.115 each; ₹0 if you send them by hand from the WhatsApp Business app.

A month is 52 weeks divided by 12. The reminder rate is the utility rate for India on Meta's WhatsApp Business Platform rate card, effective 1 October 2026; a reminder you send yourself from the app costs nothing.

Five rules

Five booking rules that cut no-shows

Confirm every booking in writing, remind the day before, ask a small advance for long or first appointments, make cancelling one message, and keep a short waiting list. The reminder is the one with trial evidence behind it; the other four are the practices we recommend because each removes a reason people don't turn up.

  1. Confirm in writing at booking

The date, time, place and what to bring, in a message the customer can find again. A booking that exists only in someone's memory is the one they forget.

  2. Remind the day before

The step the evidence supports. A text or WhatsApp message with the time and a one-word way to confirm or cancel.

  3. Take an advance where a miss hurts

Long appointments, first visits, peak slots. Say the terms at booking: what is kept if they don't come, and how late they can move the slot.

  4. Make cancelling as easy as booking

Someone who can't come and can't reach you just doesn't come. One message to cancel turns a no-show into a free slot.

  5. Keep a waiting list

Three names for the busy days. A cancellation the day before is a slot you can still fill.

Reminders on WhatsApp: the free app or the paid API

For a small clinic or salon, sending reminders by hand from the free WhatsApp Business app costs nothing and takes a few minutes each evening. Automatic reminders need the WhatsApp Business Platform, which Meta charges per message: ₹0.115 for a utility message such as a reminder, in India, from 1 October 2026.

By hand, from the app

Free. Fine up to a few dozen bookings a day. Use the app's quick replies to keep each reminder to two taps.

Automatic, through the API

Paid per message, through a provider. Worth it at volume. Meta also started charging for utility messages inside an open customer service window on 1 October 2026, per its pricing documentation.

Money down

Taking an advance by UPI at booking

In India the simplest advance is a UPI payment: a link with the amount already in it, sent the moment the slot is booked, or a Pay via UPI button on the same page the customer booked from. Nobody needs an account to pay it, and the money lands in your bank directly.

Write the terms where the customer sees them before paying: "₹200 to hold the slot, adjusted in your bill. Kept if you don't come or cancel within two hours." Clear terms are what make a kept advance fair. How UPI links work, and when you would want a payment gateway instead, is in UPI QR vs payment link vs gateway. Advances above ₹2,000 to a merchant account will carry the new UPI fee once it starts: UPI charges for merchants.

FAQ

No-shows: questions

Do appointment reminders reduce no-shows?

Yes, on the best evidence there is. A Cochrane review of trials in healthcare found text message reminders raised attendance compared with no reminder (risk ratio 1.14, seven studies, 5,841 people) and worked about as well as phone calls, for less money per attendance.

Should I charge a no-show fee?

Chasing a fee after someone failed to turn up rarely works. Taking a small advance at booking, stated plainly with your cancellation terms, does: the slot is paid for whether or not they come, and people with money down tend to call when they can't make it.

How far ahead should I send a reminder?

The Cochrane review compared reminders with none, and text with calls; it did not settle the best timing. The day before is a sensible default for most clinics and salons, with a short note on the morning for same-day slots.

What does a WhatsApp reminder cost?

Sent by hand from the free WhatsApp Business app, nothing. Sent automatically through the WhatsApp Business Platform, an appointment reminder is a utility message, and Meta's rate for India from 1 October 2026 is ₹0.115 a message.

How do I take an advance at booking?

By UPI: a payment link with the amount in it, sent after the booking, or a Pay via UPI button on the page the customer booked from. On Beeps, booking is on every plan and the UPI button is on Pro at ₹299 a year.
